Key Responsibilities:• Conduct ongoing assessments of clients' personal strengths, progress, challenges, and needs• Provide individualized assistance to clients in accessing financial, medical, and housing resources• Develop and implement critical intervention plans to address unique challenges• Maintain regular contact with hospitalized clients and coordinate their care with healthcare providers• Complete required charting and documentation, ensuring compliance with operational guidelines• Meet service and documentation requirements for programs such as Ryan White and Greene County Health Department• Assist clients in adhering to prescribed medication regimens• Facilitate access to mental health care for clients as needed

Education and/or Experience Qualifications:• Bachelor's Degree in Social Work, Nursing, or a related field (Master's degree preferred)

Additional Qualifications:• Demonstrated experience working with adults with HIV/AIDS• Valid driver's license, clean driving record, and reliable transportation• Ability to maintain accurate and up-to-date records subject to MDHSS review and inspection• Passion for making a real difference in the lives of individuals living with HIV/AIDS
